Counseling Consent Form (Minor Child)

PARENTAL CONSENT AND COMMITMENT FOR MINOR CHILD’S COUNSELING

I. PARENTAL CONSENT

I/we allow the Counselors of The Cross Church to provide biblically based counseling to my/our child named on the Counseling Intake Form.

I/we understand and have also signed The Cross Biblical Counseling Consent Form, which provides the limitations of confidentiality as it relates to my/our child.

II. PARENTAL COMMITMENT

  1. I/we understand that I/we are an important part to my/our child’s solution to his/her struggles, and as such, I/we are required to attend two parent counseling sessions without my/our child present prior to the child receiving counseling.

  2. I/we understand my/our biblical mandate to train my/our child in the Lord’s ways (Ephesians 6:4);

  3. I/we commit to being an active participant in my/our child’s counseling;

  4. I/we understand that, as the parent(s), I/we will need to sit in on my/our child’s counseling at times, as deemed necessary by the counselor;

  5. I/we understand that I/we will have my/our own counseling time separate from the child, as deemed necessary by the counselor;

  6. I/we understand that my/our own schedules might need to change to help my/our child through this counseling process;

  7. I/we commit to uphold the above points of agreement.
